Investigating Serious Incidents in Mental Health Care Services

Making your organisation a safer place - a clinically-based and practical approach

NHS Improvement will publish a new Patient Saftey Incident Response Framework (PSIRF) in Autumn 2010 for roll-out in 2020 / 21.
 

This training will help you prepare for the new SSIRF and understand how to investigate Serious  Incidents by using a Root Cause analysis process. It will help you understand your role and responsibilities, your employer’s role and contribution to your patient’s safety.

Learning Objectives

Increased understanding of patient safety in mental health care;
Appreciate why things go wrong and how to improve care - improves knowledge of humn error and human factors;
Explore safe systems and safe culture; Gain the confidence to lead RCA invetsigations, processes and applications;
Increased awareness of the principles of 'Duty of Candour' and how to fulfil your responsibiliutes;
Development of an action plan for the new PSIRF and its implementation.


Training Methods

This course is interactive and utilises a wide variety of teaching methods. Patient  Safety theory and investigation techniques are reinforced through practical use of RCA tools in a safe environment and related to a general Serious Incident scenario, but Kate draws heavily on her experiance of investigating mental health Serious Incidents over the last 5 - 10 years.
